Although the enrollment for the LOT program is free, students must complete an application form, an essay and a parental success contract is also required.

Please download either the 2012-2013 LOT Student Application or if you are a continuing LOT member, download the 2012-2013 LOT Student Recommitment Application. 

While the data may be entered into the forms electronically, the forms need to be printed and signed.  Once completed you may scan the application, and email it to LOT@columbusbmba.org or fax to 614-470-1683 by April 30th.  Thank you.

Students will be notified in May if they are selected to participate in the program.
